Conditions that Most Commonly Qualify for Hospice Care

  • Other types of Dementia (Lewy Body Dementia, Vascular Dementia, etc.)

  • Neurological disease (Stroke, ALS, Multiple Sclerosis, Parkinson's Disease)

  • Heart Disease

  • Lung Disease (COPD, Pulmonary Fibrosis, etc)

  • Liver Disease (Cirrhosis)

  • Kidney Disease (Kidney Failure)

  • other degenerative conditions

Hospice Care Team

  • The hospice care team takes care of every patient.

  • The team consists of the Medical Director, skilled nurses home health aides, advanced nurse practitioners, chaplains, licensed social workers, volunteers and bereavement support.

Eligibility Snapshot

Chesed Hospice follows the same medical eligibility as standard hospice (six-month prognosis or less, physician-certified). Services are typically covered by Medicare, Medicaid, or private insurance.
